You need a trigger, I think it's called trigger_levelchange or something like that.

Or you fire an Input like this to a point_servercommand
OnWhatever | nameofservercommand | command | changelevel *mapname* | 0.00


Or something like that, i'm on holidays, so I don't have a hammer editor here...
